DAR Continuation Sheet

A DAR continuation sheet is extra log space for the same Daily Activity Report. It is not a new shift report and it is not an incident report. It exists so officers never have to stop documenting because the first page is full.

What this form is

Busy posts — loading docks, hospitals, event venues, or overnight construction — can fill a standard activity grid before the shift ends. The continuation sheet carries the same report identity (site, date, officer, DAR number if you use one) and then provides additional time / location / activity rows.

What security professionals use it for

Supervisors want one complete story of the shift, not three untitled scraps. Continuation sheets keep overflow entries attached to the original DAR so a reviewer can read page 1, then page 2, in order. They also discourage the bad habit of erasing earlier lines to make room.

Typical information included

• Reference to the original DAR: date, site, officer, and page number (for example, page 2 of 3)
• The same time-stamped columns used on the main DAR
• Initials or officer name on each continuation page
• A note on the original DAR that additional pages are attached

When it should be used

Use a continuation sheet as soon as the last usable row on the Daily Activity Report is filled — not after the officer has already written on the back in unmarked space. If the extra writing is about a single incident rather than the rest of the shift, that usually belongs on a security incident report or a supplemental incident report, not on a continuation log.

Practical completion practices

• Number pages clearly so nothing is lost in a fax, scan, or PDF packet.
• Do not start a second DAR for the same officer and same shift just because the first form is full.
• Keep times continuous with the last entry on the previous page.
• If your company stores DARs electronically, upload the continuation with the original so they are not filed as unrelated documents.
